Hollywood smile design is the creation of a personalized smile plan by evaluating the teeth, gums, lips and facial features together.
This approach is not the name of a single treatment or a single veneer material. According to the patient’s need, teeth whitening, orthodontic treatment, composite bonding, porcelain veneers, E-max crowns, zirconia restorations, gum contouring, an implant or different restorative procedures can be part of the plan.
The aim is not to make identical, very white, copy-of-one-another teeth for all patients. In a well-planned smile design, as much as the colour of the teeth, the following are evaluated together:
- The shape of the face,
- The movement of the lips,
- The amount of tooth visible,
- The gum levels,
- The relationship of the lower and upper jaw,
- Speech,
- Chewing function,
- The patient’s age and personal features.
In some patients, only teeth whitening and a small bonding application may be sufficient. In some patients, the position of the teeth is first corrected with clear aligners, and then only a few teeth receive a minimal restoration. In more comprehensive cases with large loss of substance, wear or old veneers, crowns and other restorations may be needed.
For this reason, a Hollywood Smile should be evaluated not as a standard “veneer package” but as a personalized aesthetic and functional rehabilitation in which different disciplines work together when needed.

What is a Hollywood Smile?
Hollywood Smile is the common name of the comprehensive treatment approach that aims to create a more balanced, aesthetic and face-harmonious smile.
The expression “Hollywood” often evokes very white, even and symmetrical teeth. However, a clinically successful smile design does not consist only of whiteness and symmetry.
In ideal planning, the following elements are addressed together:
- The health status of the teeth
- The colour, shape and proportions of the teeth
- Tooth alignment
- The smile line
- The gum levels
- The position of the lips at rest and during smiling
- The facial midline
- The lower and upper jaw relationship
- Occlusal contacts
- The chewing muscles and jaw joints
- The patient’s expectations
For a smile to look natural, the teeth do not need to be completely flawless and identical to one another. Natural teeth have small differences in shape, surface and light.
Is Hollywood smile design a single treatment?
No. Hollywood Smile is the general name of a treatment plan.
One or more of the following applications may be part of the plan:

- Composite bonding

- E-max all-ceramic crown
- Zirconium crown
- Metal-ceramic crown
- Clear aligner treatment

Not every patient needs all of these procedures. The aim should be not to increase the number of treatments, but to obtain a healthy and balanced result with the most conservative procedures that are necessary.
Who can Hollywood smile design be applied to?
Smile design can be considered in patients with the following complaints:
- Those not satisfied with their tooth colour
- Those with a fracture or wear in their front teeth
- Those with a gap between their teeth
- Those with a mismatch between tooth shapes or sizes
- Those with old and aesthetically mismatched fillings
- Those who want to renew their front-region veneers
- Those who show too much gum during the smile
- Those with irregular gum levels
- Those with mild or moderate crowding
- Those with missing teeth
- Those with advanced tooth wear
- Patients with combined colour, gum and alignment problems
- Those who want a more face-harmonious and natural smile
Suitability for treatment is not determined only by the patient’s aesthetic wish. The teeth, gums, roots and jaw occlusion must be in a condition to carry the restorations.
Who may a Hollywood Smile not be suitable for?
In some patients, oral health problems need to be treated before smile design.
The situations that should be evaluated first are:
- Active gum disease
- Untreated tooth decay
- Root-tip infections
- Advanced bone loss
- Mobile teeth
- Uncontrolled bruxism
- Serious occlusion disorder
- Jaw joint complaints
- Insufficient oral hygiene
- Heavy smoking
- Unrealistic aesthetic expectations
- Patients who want intact teeth to be fully crowned unnecessarily
- People who cannot continue with regular care and check-ups
Most of these situations are not absolute contraindications. Aesthetic procedures can be planned after gum treatment, orthodontics, decay treatment or bruxism control.
Which areas are evaluated in Hollywood smile design?
A comprehensive design should not be made by looking only at a front-tooth photograph.
Facial analysis
The following features are evaluated:
- Face shape
- The vertical proportions of the face
- The facial midline
- The differences between the right and left sides
- The position of the chin tip
- Lip support
- Profile appearance
- The relationship of the lower and upper jaw with the face
The shape of the teeth should not be considered independently of the facial structure. The same tooth shape does not create the same result on every face.
Lip analysis
The following are evaluated for the lips:
- Their position at rest
- Their movement during smiling
- The amount the upper lip rises
- The relationship between the lower lip and the incisal edges
- Lip support
- The symmetry of right and left movement
A still photograph does not always show the natural movements of the lips. Therefore, in necessary cases, video recordings during speech and smiling can be used.
Dental analysis
The following elements are examined:
- The colour of the teeth
- Length and width proportions
- Tooth shapes
- The contacts between the teeth
- The midline
- The level of the incisal edges
- Surface texture
- Tooth alignment
- Wear
- Fractures
- Old fillings and veneers
Gum analysis
The following are evaluated for the gums:
- Bleeding
- Swelling
- Recession
- Differences in gum levels
- The amount visible during the smile
- The papillae between the teeth
- Gum thickness
Occlusion analysis
For the long-term health of aesthetic restorations, the contacts of the lower and upper teeth should be examined.
The following are evaluated:
- The contact of the front teeth
- The force distribution of the back teeth
- The forward and lateral movements of the jaw
- Front-tooth guidance
- Deep bite
- Open bite
- Crossbite
- Clenching and grinding
- The chewing muscles
- Jaw joint findings
In restorations that look beautiful during the smile but are not compatible with the occlusion, fracture, detachment, wear or muscle and joint complaints can develop.
What records are taken before a Hollywood Smile?
According to the patient’s need, the following records can be used:
- Intraoral examination
- Periodontal evaluation
- Intraoral and facial photographs
- Smiling and speech videos
- Digital intraoral scanning
- Lower and upper jaw occlusion record
- Panoramic radiography
- Intraoral radiographs
- Three-dimensional imaging in necessary cases
- Examination of the chewing muscles and jaw joint
These records are used not only for the aesthetic design, but to evaluate the suitability of the teeth and surrounding tissues for treatment.
What is digital smile design?
Digital smile design is a planning method that helps evaluate the facial, lip, dental and gum records together in a digital environment.
During the digital design:
- The possible lengths of the teeth,
- Their widths,
- The midline,
- The smile curve,
- The gum levels,
- The relationship with the lips,
- Colour and shape options
can be visualized.
Digital Smile Design systems can facilitate patient–dentist–laboratory communication and help make the pre-treatment design more understandable. Current systematic evaluations report that digital smile design can contribute to aesthetic planning and patient satisfaction. However, the digital image is not a guarantee of the clinical result and does not replace a biological or functional examination.
Does the digital design show the result exactly?
No. Digital design is a planning and communication tool.
The image created on the screen cannot fully predict:
- Gum healing,
- The real light properties of the material,
- Lip movements,
- Occlusal forces,
- The response of the soft tissues to treatment,
- The patient’s biological healing.
For this reason, digital design should not be presented as a “definitive result simulation”.
What is a mock-up?
A mock-up is the trial of the planned tooth shapes in the patient’s mouth with a temporary material.
With a mock-up, the following can be evaluated:
- The length of the teeth,
- The smile line,
- Lip support,
- Speech,
- Harmony with the face,
- The planned volume.
It can help the patient see and feel the design in their own mouth, not just the image on the screen.
A mock-up is also not an exact guarantee of the permanent result. However, it allows the plan to be evaluated before the permanent tooth preparation.
Are the teeth cut in Hollywood smile design?
Tooth cutting is not done in every patient. The amount of cutting varies according to the procedures to be applied.
Procedures that may not require tooth cutting
- Teeth whitening
- Orthodontic treatment
- Some composite bonding applications
- Selected no-preparation veneers
- Professional cleaning and surface adjustments
Procedures that may require minimal preparation
- Porcelain veneer
- E-max veneer
- Inlay and onlay
- Partial ceramic restorations
Procedures that may require more extensive preparation
- Full E-max crown
- Zirconium crown
- Metal-ceramic crown
- Full crowning of teeth with large restorations or fractures
Both the statements “in a Hollywood Smile the teeth are never cut” and “all teeth are reduced” are wrong.
The aim is to choose the most conservative treatment that meets the clinical requirement.
Do intact teeth need to be crowned?
In most patients, it is not necessary to reduce all intact teeth for a full crown only for aesthetic purposes.
The following options should be evaluated first:
- Teeth whitening
- Clear aligners or orthodontics
- Composite bonding
- Porcelain veneer
- Gum treatment
- Limited restorations
Nevertheless, a full crown may be needed in case of advanced discolouration, large fillings, fractures, wear or reduced structural support.
Treatment should be planned not on the basis of “how many teeth will be crowned”, but according to the separate biological and structural need of each tooth.
What procedures can a Hollywood Smile include?
Teeth whitening
If the shape and alignment of the teeth are suitable, only the colour problem can be corrected with professional teeth whitening.
Whitening is one of the first options that preserves natural tooth tissue. It does not lighten the colour of veneers and fillings.
Orthodontic treatment
If the main problem of the teeth is position and crowding, clear aligners or fixed orthodontic treatment can be considered.
Orthodontics:
- Moves the teeth together with their roots.
- Can preserve sound tooth tissue.
- Can adjust the occlusion.
- Can reduce the number of restorations to be done later.
In some patients, only whitening and small bonding procedures are sufficient after orthodontics.
Composite bonding
With tooth-coloured composite bonding material:
- Small fractures,
- Limited gaps,
- Small shape differences,
- Incisal edge wear
can be corrected.
Bonding is mostly a less invasive and repairable option. However, it can stain and show surface wear more than porcelain over time.
Porcelain veneer
It is a thin ceramic restoration mostly bonded to the front surface of the tooth.
In suitable patients, it can help preserve more enamel tissue than a full crown. Not every patient is suitable for a no-preparation porcelain veneer.
E-max veneer
It is a lithium disilicate-based all-ceramic restoration. E-max can be used in the form of a crown or a veneer.
It can be considered especially in single front teeth and in cases where natural light properties are important.
Zirconium veneer
It is a zirconia-based restoration with high mechanical durability.
It can be considered in teeth with large fillings, reduced structural support, a dark substructure or higher force. It is not automatically the best aesthetic option for every front tooth.
Gum aesthetics
Irregularity in gum levels or an excessive gum appearance can arise from different causes:
- Gum overgrowth
- Teeth appearing short
- The upper lip rising too much
- Vertical maxillary excess
- The eruption position of the teeth
- Wear
- Altered passive eruption
Treatment does not consist only of cutting the gum with a laser. According to the cause, gingivectomy, crown-lengthening, orthodontics or different approaches may be needed. In advanced gum problems, a periodontal evaluation may be required.
Implant and missing-tooth treatments
Missing teeth can be completed with an implant, a bridge or different prosthesis options.
The position of the implant, the gum level and the prosthesis emergence profile affect the aesthetic result, especially in the front region.
How many teeth are done in a Hollywood Smile?
There is no fixed number.
The scope of treatment is determined according to the following elements:
- The number of teeth visible during the smile
- Upper and lower lip movements
- The colour of the existing teeth
- Whether the problem is in only a few teeth or in the whole smile line
- The type of treatment to be used
- The need to preserve intact teeth
In some patients, bonding on only two teeth is sufficient. In some patients, the upper front six or eight teeth may need to be included in the aesthetic planning. The lower teeth, premolars or the whole occlusion should be added to the treatment only if there is a clinical requirement.
The approach “20 or 24 teeth must be crowned as a fixed number” is not compatible with the principle of personalized treatment.
Can a Hollywood Smile be done only on the upper teeth?
In some patients, adjusting only the upper smile line may be sufficient.
However, the lower teeth should also be evaluated in the following situations:
- If the lower teeth are markedly visible during the smile
- If a marked difference will occur between upper and lower tooth colours
- If the position of the lower teeth affects the occlusion of the upper restorations
- If there is advanced wear
- If the occlusal height will be rebuilt
- If there is bruxism
The lower teeth do not necessarily need to be crowned. More conservative options such as orthodontics, whitening or limited bonding can be used.
How is the tooth colour chosen in a Hollywood Smile?
Colour selection is not just determining the whitest shade.
In the evaluation, the following are taken into account:
- The patient’s skin colour
- Eye and hair colour
- Age
- Lip colour
- Neighbouring natural teeth
- Facial structure
- Personal preference
- The material to be used
- The restoration thickness
Very white teeth can provide the desired appearance in some patients; but they may not look natural on every face.
In a natural restoration, instead of a single colour there can be light and colour transitions between the neck region, body and incisal edge.
Does a Hollywood Smile look natural?
With correct design and material selection, natural-looking results can be aimed for.
The main factors affecting naturalness are:
- The harmony of the teeth with the face
- Correct length and width proportions
- Small differences of the teeth from one another
- Surface texture
- Colour transitions
- Incisal edge features
- Gum levels
- Lip movements
- The light transmission of the material used
- Laboratory customisation
Preparing all the teeth the same length, a single colour and excessively flat can create an artificial appearance.
Does everyone look the same with a Hollywood Smile?
No. The aim of personalized planning is not to apply the same standard smile to every patient.
The shape of the teeth should be determined according to:
- The face shape,
- Age,
- The lips,
- The existing natural tooth features,
- The patient’s aesthetic preference.
Instead of rigid templates such as “female teeth” or “male teeth”, the patient’s individual facial features and expectations should be taken into account.
Does a Hollywood Smile change the face while changing the smile?
The length, volume and position of the teeth can affect lip support and the facial expression during the smile.
However, dental restorations basically do not change:
- The real position of the jawbone,
- The nose structure,
- The chin tip,
- The skeletal proportions of the face.
In patients with a marked jaw retrusion, protrusion or facial skeleton problem, different treatments such as orthodontics or orthognathic surgery may be needed.
Does a Hollywood Smile correct the occlusion?
A Hollywood Smile is not an occlusion treatment on its own.
During a comprehensive restorative rehabilitation, the occlusal contacts can be reorganized. However, skeletal jaw disorders or marked tooth-position problems should not be corrected only with veneers.
According to the type of occlusion problem, the following may be needed:
- Orthodontics,
- Clear aligners,
- Orthognathic surgery,
- Restorative rehabilitation,
- A jaw joint evaluation.
Does clenching affect a Hollywood Smile?
Yes. Bruxism can increase the mechanical forces on the restorations.
In patients who clench their teeth, the following problems can develop:
- Porcelain fracture
- Veneer detachment
- Composite wear
- Screw loosening
- Natural tooth or root cracking
- Wear on the opposing teeth
- Muscle pain
- Jaw joint complaints
For this reason, tooth wear, muscles, jaw movements and occlusal relationships should be evaluated before treatment.
In patients who need it, the following can be planned:
- Occlusion adjustment,
- Changing the material and restoration design,
- A night guard,
- Bruxism and sleep evaluation.
Using a durable material does not eliminate all the risks of bruxism.
Does a Hollywood Smile affect the jaw joint?
When the shape of the teeth and the occlusal contacts are changed, the chewing system can also be affected.
Especially in cases where many teeth are restored, the following should be evaluated:
- Existing jaw joint complaints,
- Mouth-opening movement,
- Muscle tenderness,
- Lower jaw position,
- Occlusal height.
It cannot be said that smile design definitively treats jaw joint diseases. However, comprehensive occlusion changes made without taking existing joint and muscle problems into account can make it difficult for the patient to adapt.
How is Hollywood Smile treatment done?
1. Evaluation of the patient’s expectations
The patient explains which features they are bothered by and what kind of change they want.
The aim is not to direct the patient to a specific veneer package, but to determine their real need.
2. Clinical and radiological examination
The teeth, gums, roots, bone structure and occlusion are evaluated.
If there are active diseases, they are treated before aesthetic procedures.
3. Photographs, video and digital records
Facial, lip, smile and intraoral records are taken. The current situation is documented with digital scanning and occlusion records.
4. Comparison of treatment options
The options that can be applied to the patient are explained:
- Whitening only
- Orthodontics
- Bonding
- Veneer
- Crown
- Gum treatment
- Combined approach
The advantages and limitations of the more conservative option are explained.
5. Digital design and mock-up
In necessary cases, the planned teeth are designed digitally and tried temporarily in the mouth.
6. Preparatory treatments
If procedures such as gum treatment, whitening, orthodontics, root canal treatment or an implant are needed, they are applied in a suitable order.
7. Restorative applications
According to the treatment plan, bonding, veneer, E-max or zirconia restorations are prepared.
8. Try-in and customisation
Colour, shape, length, speech, lip support and occlusion are evaluated.
9. Permanent bonding or fixation
The bonding protocol suitable for the restoration type is applied.
10. Check-up and care
The occlusion, gum health and cleaning are monitored regularly.
How long does Hollywood Smile treatment take?
There is no fixed treatment time.
The main factors affecting the time are:
- Whether only whitening or bonding is done
- The number of porcelain veneers or crowns
- Gum treatment
- Orthodontic preparation
- Implant requirement
- Decay and root canal treatments
- Temporary restorations
- The number of try-ins
- Laboratory production
- The patient’s healing process
Limited bonding or whitening can be completed in a short time. Porcelain restorations require more than one appointment. In cases with orthodontics, an implant or gum surgery, the process can take months.
For this reason, it is not correct to tell all patients that a Hollywood Smile is “completed in 4–6 days”.
Is a Hollywood Smile painful?
The level of discomfort varies according to the procedures done.
Temporary sensitivity can occur during whitening. Procedures requiring tooth preparation can be done under local anaesthesia. After gum surgery or implant applications, pain, swelling and sensitivity can be seen during the healing period.
Definitive statements such as “it is completely painless” or “no discomfort occurs” should not be used.
What are the advantages of a Hollywood Smile?
The advantages that a well-planned treatment can provide are:
- Adjustment of the tooth colour
- Improvement of tooth shapes and proportions
- Repair of fractures and wear
- Closure of small gaps
- Balancing of the smile line
- Adjustment of gum levels
- Completion of missing teeth
- Renewal of old restorations
- Support of chewing and speech in suitable patients
- Creation of a smile more in harmony with the person’s facial features
Not all of these advantages can be obtained in every patient, and the results depend on the individual clinical conditions.
What are the risks and limitations of a Hollywood Smile?
According to the procedures applied, the following risks can be seen:
- Irreversible tooth cutting
- Temporary or permanent sensitivity
- The need for root canal treatment
- Porcelain fracture
- Veneer detachment
- Composite discolouration
- Decay under the veneer
- Gum recession
- Gum inflammation
- Colour or shape dissatisfaction
- Difficulty adapting to speech
- Occlusion incompatibility
- Complications due to bruxism
- The need for maintenance or renewal of the restorations
- Loss of excess tooth tissue due to unnecessary treatment
Being for aesthetic purposes does not mean the treatment is risk-free.
Is a Hollywood Smile permanent?
Because a Hollywood Smile does not consist of a single material, a fixed period of use cannot be given.
Whitening can be repeated over time. Composite bonding may require maintenance and polishing. Although porcelain veneers and crowns can be used for a long time, they can break, detach or need to be renewed. Changes can occur in the gums and natural teeth over time.
The elements affecting the period of use:
- The treatments applied
- The remaining natural tooth tissue
- Material selection
- Bonding and production quality
- Oral hygiene
- Gum health
- Bruxism
- Smoking
- Eating habits
- Regular check-ups
Statements such as “10–20 years guarantee”, “lifelong permanent” or “never requires another procedure” are not scientifically correct.
How is care done after a Hollywood Smile?
Care should be personalized according to the restorations applied.
In general:
- The teeth should be brushed at least twice a day.
- The spaces between the teeth should be cleaned with dental floss or an interdental brush.
- The area under bridge and implant prostheses should be cleaned with special tools.
- Very hard objects should not be broken with the teeth.
- Nails and pens should not be bitten.
- If there is bruxism, a night guard should be used.
- Gum bleeding should not be neglected.
- Smoking should be reduced or stopped.
- Regular check-ups and professional cleaning should be done.

Because a Hollywood Smile is not a single procedure, a standard package price does not reflect the real need of every patient.

Before treatment, the following should be clearly planned:
- How many visits are required,
- The temporary-tooth process,
- The healing periods,
- The try-in requirement,
- The check-up appointments,
- Possible maintenance and repair procedures.

Personalized, natural and conservative smile design
In Hollywood smile design, the aim is not to crown all the teeth the same way or to use the whitest possible colour. A good smile design should be addressed as a personalized aesthetic dentistry approach.
In correct planning, the order of priority is as follows:

- Preserving natural tooth tissue as much as possible
- Evaluating the tooth and jaw positions

The treatment decision should be made by evaluating together the clinical and radiological examination, the facial and lip analysis, the amount of existing enamel and dentin, gum health, jaw occlusion, bruxism and the patient’s expectations.
